Parent/Athlete Notices

Before an athlete can try out for any sport, they must have a Central Unified Co-Curricular Clearance/Eligibility Packet on file. This packet must be completely filled out along with a  current sports physical signed by a doctor (physical must be within the last 12 months to be valid), and medical insurance information on file.
